MD or DO, BC/BE Internist, Geriatrician, or Family Practitioner with Hospice & Palliative Medicine being a desirable distinction - Subacute Skilled Nursing Facility / Geriatric Palliative Care.
Position Summary:
We are seeking a compassionate Physician to join our subacute Skilled Nursing Facility (SNF) Transitions Team, focusing on geriatric palliative care and goal-concordant care. This role is perfect for a Physician with prior hospitalist or SNFist experience who is looking for a meaningful career with exceptional work-life balance.
Key Highlights:
• Monday through Friday schedule - enjoy your weekends and evenings!
• Rotating (2:30) shared Group call coverage from home using internal Nurse Coordinator triage • Supportive interdisciplinary team (IDT) collaboration with physicians, nurses, therapists, social workers, Pharm D, and others.
• Focus on whole-person wellbeing - addressing not just medical, but emotional, psychosocial, and spiritual needs.
Core Responsibilities:
• Provide high-quality subacute and palliative care management for complexly ill participants admitted to contracted SNFs - census of 20-25 patients, averaging 5 admissions +/- discharges per week, and allowing for hybrid on-site/remote work with bedside rounds typically 2-4 times per week.
• Lead and participate in IDT meetings to develop and implement individualized, participant-centered care plans frequently addressing palliative and end-of-life needs.
• Engage in meaningful advance care planning and goals-of-care conversations.
• Coordinate transitions of care to optimize outcomes and respect participant preferences.
• Partner closely with families, SNF teams, and the larger PACE clinical team to ensure seamless, compassionate care.
• Contribute to ongoing quality improvement initiatives and a culture of kindness, respect, and excellence with all partners.
What We're Looking For:
• BE/BC Physician licensed in COLORADO with active DEA registration.
• Minimum 2 years of experience as a hospitalist, SNFist, or in post-acute/long-term care settings.
• Expertise in geriatric syndromes, chronic disease management, palliative care, and interdisciplinary care models.
• Passion for person-centered, holistic care and commitment to working collaboratively within an IDT.
• Strong communication and leadership skills; bilingual ability is a plus.
