Summary:
Penrose Main Hospital (PEN) is looking for a locum vascular surgeon to help provide the team with coverage December 2024. Call coverage for vascular surgery includes surgical treatment of blunt and other vascular trauma.
BID DUE DATE: First to Fill! Rate Caps for Submission:
- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- On Call (Daily) xx4795 and below Per 24 hours to include 8 hours gratis
- 24 Hour Holiday Call Rate Must be 1.5x On Call (Daily) rate xx7192.50 and below Per 24 hours to include 8 hours gratis
- Call Back xx425 and below Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms
- Holiday Call Back Must be 1.5x Callback rate xx637.50 and below Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms
Average Call Back: 47 Hours Schedule/Availability Requirements: Details: - 24 hour call to include 8 gratis hours.

Credentialing Information: - Credentialing timeframe: 6090 days but typically much less time if everyone is responsive. Site prefers to complete within 30 days.

EMR Required Training:
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual training & Onsite training Only able to attend if Modules are completed before training. Virtual EMR training is schedule before the providers first day. If onsite training is required it is typically looped into the morning of their first day.

Requirement description :
**MUST NOTATE ALL DURING NAME CLEAR TO AVOID BEING REDIRECTED**- Board Certification Vascular Surgery Required
- Active CO license Required or IMLC (LOQ in hand at time of submission)
- Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
- Must disclose # of shifts per month ongoing provider can offer to the team at submission Required
- Candidate must be available for any of the stints detailed in job description to be considered Required
- Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
